We need a dedicated and skilled Triage Nurse (RN) to join our innovative telehealth team. You will play a vital role in assessing patient needs, providing guidance, and coordinating care as part of our comprehensive healthcare solution. Collaborating closely with other healthcare professionals, you'll help streamline the patient experience and ensure that they receive timely and appropriate medical care.

Responsibilities

• Conduct thorough assessments to determine the urgency of patient conditions and advise patient next steps
• Communicate effectively and empathetically with patients regarding their health concerns and treatment options
• Document patient interactions accurately to ensure continuity of care and compliance with protocols
• Collaborate with interdisciplinary team including providers, nurses, medical advocates and mental health team to optimize patient care and improve health outcomes
• Provide education and resources to patients regarding their symptoms, conditions, and health goals
• Utilize critical thinking and clinical judgment to prioritize incoming inquiries and triage accordingly
• Maintain a compassionate, patient-centered approach in all interactions

Requirements

• Hold a valid and unrestricted Registered Nurse license
• Bring at least 5 years of experience in a clinical setting in urgent care, emergency room or a busy primary care setting
• Demonstrate exceptional clinical judgment and critical thinking skills
• Exhibit strong communication skills and the ability to connect with patients
• Have proficiency in utilizing telehealth technology and electronic medical record systems
• Ability to work independently while effectively managing multiple patient inquiries
• Bilingual in Spanish/English – preferred

Pay Rate is $35-$45/hr - depending on experience
